Mail Order Form
Our Mail Order Form is for ordering by "snail" mail. You can order by check or money order using this form and also by credit card. The Mail Order Form can be used to place orders in 2 ways:
The first way is to simply print the Mail Order Form and fill it in with item(s) you are ordering from our site as you shop.
The second method is to order items by clicking the "ADD TO CART" button (just as if ordering by credit card) until you've ordered everything you are looking for. Then just print your shopping cart page(s) and include it with the Mail Order Form when you mail in your order. If you use the second method and print the shopping cart page(s) then you do not need to fill in the middle portion of the Mail Order Form since your ITEM(S), QUANTITY, PRODUCT NAMES & PRICES are on the print out of your shopping cart. Just take the TOTAL from the bottom of the shopping cart page and add it to the Mail Order Form then fill out the rest of the form and mail them in with your check or money order.
The attributes column on the order form is just for items that ask you to choose a certain color or size. There are very few products at the moment that have attributes to choose from so most people will not use this column. If the item has attributes to choose from you would see the choices in the items description.
